It is vital. Decisions are made based on past actions. If one doctor is unaware of what has been done thus far problems could arise.

If there's more than one care giver, then an up to date record is vital. To prevent double and even triple medications, what was given and when, what procedures taken, etc... Even one caretaker must keep up with his/her on procedures as in every case they would most likely have more than one patient.
