Effective communication cannot be achieved without all parties possessing and applying the following qualities/skills:
Respect.
Honesty.
Listening. (as opposed to just "hearing." There is a difference)
Clarity.
An open mind.

If anyone involved in the communication is lacking in any of these areas, you are going to have problems.
