In the U.S. there are supposed to be yearly unannounced visits to inspect nursing homes. But usually word leaks out ahead of time when they are coming.
As is the nature of bureaucracies everywhere, there are often family connections between those running nursing homes and those on the regulatory side. Protecting helpless nursing home patents from neglect and abuse gets more lip service than action and a lot of regulations are ignored.
As long as most of the kids and grand-kids of the patients in those homes don't really care, nothing will change. They few who do care are easily ignored.
