What can you do for someone who is TERRIFIED to drive on the interstate?

I have a few ideas.
Have them ttend the Bondurant racing school, specifically the Highway Survival Training course.
Have you considered taking them to a few driving lessons again? Having a professional beside you talking them through things and giving them safety tips may help them regain confidence.
And lastly
I think that first you should get them used to driving on the interstate without the onramps. There are certain places where you don't have to merge at all - the on ramp becomes an off ramp after about half a mile. Ask your interstate driving friends about where one of these exist locally. It also helps to have a friend to talk them through it. Just make one of these short trips a few times, then slowly ramp it up.

Face your fears with a confident driver sitting to your right! Practice until it becomes routine for you! It can be scary, but being afraid is more of a hazard than the driving! If you must drive on the interstate on a regular basis, you don't have much of a choice, but to keep doing it until the fear subsides. If you don't have to drive on the interstate, then don't. Practice when traffic is at its' lowest ebb, then work your way up to the heavier traffic. Good luck, and stay safe!

Enroll them in driving school and let the instructor take it from there. He or she will know how to deal with them and their fears. Reminder some driving schools vehicles have that extra brake on the passenger side.

Wow, tough one. Does this person have a reason to fear driving on the interstate?
I was in a three car accident back in 1992. A car involved in a "road rage" incedent with another car was almost stopped in the center lane of a five lane road. He pulled into my lane as I started to pass at almost seventy miles per hour. I spun and was then hit by another car. the other car spun me again and I hit the center divider. I looked up and the car that caused the accident was leaving! My car was still drivable, so I chased him down and ran him onto the grass beside the right lane. The other car involved in the crash then blocked the front of his car until the police arrived. It turned out it was an eighteen year old kid and he was just scared so he ran. His insurance paid for everything. My car alone was over six thousand dollars.
Two days later I went out to drive again. I knew something was off as soon as I got on the highway I felt like all the other cars were moving at me. I had to take side streets. I was able to work my way back onto the highway, but it took a few weeks.
I now drive one of the quickest cars built. It was made for the track and that's where I drive it when I want to go fast. On the street I still watch the "other guy" and make sure that I don't cause a problem.
If the person simple cannot work through the problem on their own, they may be able to work with an instructor and learn how to cope with traffic. Check out something like this. They may be able to find private lessons.

It is good to have a little fear of this, but a person has to function in society.
For external help there is hypnotherapy. Sometimes this works great, sometimes not. There are medicines that can be taken. They work by aiding the person in helping themselves. A driving course is not likely to work because there is no reason to suppose the instructor has any training in dealing with this.
Systematic desensitization is what is usually recommended. This is where a person drives in more and more traffic and approaches the freeway closer and closer until the fear is overcome. One must first be taught relaxation skills in order to control fear and anxiety responses. There are several of these. One is to tighten and loosen the muscles in order to burn off the adrenaline that the anxiety has brought on. Another is to start at one end of the body and concentrate on relaxing it and then move progressively. There are breathing exercises and the elimination of negative thoughts. You want to recognize the attack of fear and perhaps measure it on say a scale of 1 to 100. This helps one to realize that the fear is not always the same amount. As progress is made congratulate yourself. And also avoid caffeine
.This is often classed as a type of agoraphobia or a fear of going outside. That may need to be dealt with also.

First Step is to enroll in a GOOD driving school and learn not to panic at any situation which may occur[includes minor to major accidents] while driving.When you panic only the situation will get worse.
I have heard a real happening where a person panicked while stopping the car and pressed accelerator instead of break ,resulting an accident.Be cool.
First make yourself comfortable while driving in lesser traffic.
May be WATCHING A FEW RACE CAR FILMS would help too.Its just your mentality which makes you terrified.Watching films where hero does unbelievable stunts may help some people to over come fear,as almost everyone place themselves as the hero[unknowingly/subconsciously]while watching films.Thats why audience also cry while watching sorrow films although they knew that everything is only acting.

O well, I used to be in this boat. How did I overcome the fear? I just stay off the interstate until I was ready. When the fear was lifted from me a little, I just called on the name of "Jesus" to put that fear behind me. I drove in the "right lane" all the time until one of my tires went flat in the rush hour traffic. Now I was scared and stranded on this interstate, Again, I pray to my spiritually father for guidance. Someone stopped and help me. I got back into the car and "speed" all the way home on that interstate. After that day, I had no more FEAR in driving on the interstate. With that be said, the person should have FAITH in its driving or stay off the interstate completely.

All I can say it practice, fear is natural in human beings. It goes away with time.
If not, then you should practice doing something way harder than this, just as Jack (psynopsis) said, some kind of racing school. Then they wouldn't feel scared at all, but don't go over the speed limit :P

Ok i would reccomend that you first drive with someone you personally know that is really close to you like you're dad or somethingor make them do it a couple times to learn then you drive with him on the side instructing you then have to do this about 2 times a day for at least 2weeks then when you're in the real deal you wond be terrified at all you will just remember what you did and what you learned then voula you did it :)!

Drive for them or tell them to take a taxi. That's all we need is another freaked out nervous nilly on the interstate.
Incase you are wondering, I am only joking, I actually agree with the rest of the gang here, have them see a psyciatrist or take a driving course.
