have shared agreement, he to provide insurance, have letter from ins. company it was terminated 3.1.08 sent him certified letter requesting he still does not respond

You may have to go back to court to get it. You will get a judgment and if he doesn't provide it he will be held in contempt of court. He can be jailed or sometimes the money for it taken out of other income Time to see your friendly lawyer.
