Can a bank open loan in your name without your signature or knowledge ?

Had to declare bankruptcy 6 years ago and decided to keep our home which we were lead to believe was on a secured loan, so we continued to pay the bank. A year after being released from bankruptcy we found out the loan was actually unsecured. The bank had to take the money we had paid up to that point and pay it to the trustee and we continued to pay the trustee to the end of the loan term and now own the home.
I was checking my credit history for the first time since the bankruptcy the other day and and found information I was not aware of. The Bank had actually closed the origional loan and deemed it as paid up to the point the loan was declared as unsecured then they opened a new loan to pay the trustee under my name without notification or knowledge. There is now a loan showing on my credit history that I wasn't aware of. Is this procedure the norm or legal ?

2] what kind of papers did you sign when you opened the account? was there something in fine print?
3] what is the term of filing a complaint for recovery in your country?
